Follow the procedure for replacement of a broken injector with the universal injector.

Understanding why this might happen

OK – so you’re moving fast, trying to get those cartridges filled.  You can’t find the hole you drilled when you are ready to fill.  So what do you do?  You force the injector needle down and break it.  OR maybe clog it with pieces of plastic.   The picture below shows the most common example of when this might happen.

When using the T1 adapter with an ink tank cartridge (shown below) sometimes the cartridge may move back/forth in the cradle a bit causing the hole to not be lined up properly.  If you force the injector needle down too hard you can break it.
